Fusion delivers strategic developments in social care both as specific projects for Councils and also in the combination of health and social care strategic development. Fusion contributes to projects considering holistic care, the ability for people to be cared for in their own home and the efficient and effective use of public estate as a whole, for example developing the "one public estate principle".

Our roles include developing the strategic integration of community services associated with health and social care, including the continuing rise of the voluntary sector and other service providers and we have been leading on a number of strategic estates plans which integrate such thought.

 

Additionally we can develop bespoke models which identify the future demand for health and social care requirements within the community and have also modelled future requirements for care homes, extra care housing and domiciliary based care requirements.
